Toes take a surprising amount of punishment inside a shoe. The tops rub against the upper, the tips press into the front, and nails get knocked with every downhill step. You do not always notice it happening, but you notice the sore spot, the thickened skin or the bruised nail that follows.

Toe Protection or Toe Alignment?
These two ranges are often confused, and buying the wrong one is the commonest mistake people make. The difference is straightforward once you know it.
Toe alignment products go between your toes. They create space where toes are crowding, overlapping or pressing against one another, and the problem they solve is one of position.
Toe protection products go over or around your toes. They cushion the toe itself against something external, usually the inside of your shoe, and the problem they solve is one of contact.
So the question to ask is simple: what is your toe rubbing against? If it is another toe, you need alignment. If it is your shoe, your sock, or the ground, you need protection.
The pattern of where it hurts usually gives you the answer. Soreness on the top of a toe, or a corn forming there, means the shoe upper is pressing down, which is common when a toe sits in a bent position. Pain at the tip or under the nail points to the toe hitting the front of the shoe. Rawness on the outer edge of the little toe means the side of the shoe is the culprit. All three call for protection.
Plenty of people need both, of course. A toe that has been pushed out of position by crowding often ends up rubbing on the shoe as well, and that is a case for a spacer and a cushion working together.
Choosing the Right Toe Protection

Gel Toe Caps
Closed at the end, so they cover the whole toe including the tip and nail. The best choice when your toe hits the front of the shoe, when a nail is sore or bruised, or when the tip has become tender.

Toe Sleeves and Tubes
Open at both ends, so they cushion the length of the toe while leaving the tip free. More breathable than a cap and ideal for corns on the top of a toe or general rubbing along its length.

Hammer Toe Cushions and Crests
These sit underneath the bent toes and gently support them from below, which reduces the pressure driving the tip into the sole and the knuckle into the shoe upper. Made for claw, hammer and mallet toes.

Toenail Protectors
Extra padding over the nail itself. Worth having if you run or walk downhill regularly, if you have lost nails before, or if a thickened or damaged nail is sore under pressure.

Little Toe Protectors
Shaped for the fifth toe, which sits at an angle and takes the brunt of the side of the shoe. A standard sleeve rarely stays put there, which is why the little toe needs its own design.
Getting Toe Protection to Stay Put and Last
Gel toe products work well when they fit properly. A few points will save you frustration.
Size matters more than with most products. Too loose and it slides down as you walk; too tight and it restricts circulation to the toe. It should stay in place through a normal day without leaving a mark when you take it off.
Put them on dry feet. Gel grips far better against dry skin, and a light dusting of foot powder helps in warm weather when feet perspire more.
Check your shoes have room. Adding a gel cap takes up space at the front of the shoe, which is exactly where space is usually shortest. If your shoes are already snug at the toes, a slimmer sleeve will serve you better than a full cap.
Wash and dry them daily. Rinse in warm soapy water, dry properly, and they will last considerably longer. Gel left damp inside a shoe is not doing your skin any favours either.
Do not wear them over broken skin. If the toe is raw, blistered or open, cover the wound with a dressing first. Gel directly against damaged skin traps moisture and slows healing.
Look at the shoe as well as the toe. If the same toe keeps needing protection, the shoe is telling you something. A wider toe box or a half size up often removes the problem entirely.
